  • Grant Submission Deadline: February 14, 2025 Midnight

  • Project Description:

       Please provide a description for each of the five segments below.
  • Provide a description of the project and how it will impact the students. Photographs and/or line drawings should be included where appropriate. (20 points)
  • If funded by the Arts Society, what is the intended time period for implementation. (20 points)
  • Describe the impact the project will have in supporting student scholarship, the number of students served, and the long term improvement to the facilities and/or teaching program that will accrue to the institution by receipt of this grant. (20 points)
  • Specify amount requested through this grant, total project cost, and expenditure estimates if project is multi-faceted. (20 points)
  • If the project requires "Matching Funds", detail the availability of those funds and their source. (20 points)

Accounts Payable Procedure

Donors have the ability to restrict the use of their contribution. Each Foundation account has a purpose or restriction. Before submitting a Payment Request Form, please check to make sure the request for payment meets these restrictions. The deadline for payment submission to MWSU Foundation is 2 p.m. Wednesday in order to receive a check by Monday after 10 a.m. Each request must have the following items to ensure that it is processed during the week:
  1. MWSU Foundation Payment Request Form.
  2. Request signature and approval signature from the Dean, Vice President, or authorized person in charge of the Foundation account.
  3. Invoice or documentation for request.
  4. W-9 form or W-8ECI (for international vendors) if payment to outside Vendor.
Checks are released to individuals on the following Monday after 10:00 a.m. in MWSU Foundation Office, Spratt 110. If you have any questions regarding procedures or forms, please contact the MWSU Foundation Accountant at 816-271-5653.
